At Opeeka our mission is to promote well-being for all. Systems of care is a service delivery approach that builds partnerships across service sectors to create integrated care for meeting a family’s variety of needs. The overarching goal is to improve well-being for children and youth within families. The ideas for a system of care approach date to the mid-1980s. However, adoption has been challenging due to siloed information systems and competing program priorities. Without coordinated care, challenges arise during care for children and youth, such as: long stays in out of home care, increased offending, or entry into child welfare or juvenile justice.
Opeeka built Person-Centered Intelligence Solution (P-CIS) to unify care and promote well-being. With P-CIS, people can choose to allow providers in the system of care to share relevant information about an individual or family’s story and progress in care. In addition, people can track their own progress across all services in one place. As pre-authorized by people and programs, P-CIS shares information on just what is needed, at just the time it is needed. Agencies in a system of care can work together to improve the well-being of every individual efficiently and effectively.
