An Opeeka customer offered 56 different service types for adults with substance use disorder. Care staff worked with clients to establish a personalized care plan. Care administrators were able to see in P-CIS that clients who received “Employment Training” services as part of their care plan were over 8 times more likely to succeed than those who did not receive those services. Administrators could see information about who responded well to which service types and could take action to ensure services were matched based on client’s specific circumstances, increasing overall likelihood of success for all clients in care.

Person-Centered Intelligence Solution (P-CIS, pronounced “pieces”) works as a stand alone care management system or connects to existing electronic records to infuse care tracking and monitoring tools. P-CIS tracks patient progress on any domain of well-being from any data source (EHR/EMR, text, email, tablet, passive monitoring device, apps, and more). It merges current and historic data into 360-degree views of whole-person wellbeing (call “Story Maps”), and measures impact of care and progress over time (called “Trajectories of Success”). P-CIS allows care agencies to implement value-based care, real-time patient monitoring, and patient reported outcomes without disrupting their current technology or workflow.
